Why is a Laser Excision of Nasal Granuloma Performed?

Nasal granuloma excision is recommended for the following scenarios:

  • Persistent symptoms: Surgical removal of nasal granulomas may be recommended if conservative management fails to alleviate persistent symptoms, such as nasal congestion, epistaxis (nosebleeds), facial pain, or altered sense of smell.
  • Failure of medical treatment: If conservative treatment measures like corticosteroids or antibiotics fail to provide symptomatic relief.
  • Risk of complications: Surgical intervention is required if nasal granulomas pose a risk of complications, like nasal obstruction resulting in breathing difficulties, or frequent infections.
  • Treatment of Underlying Cause: Excision may be part of a comprehensive treatment plan aimed at addressing the underlying cause of the granuloma, such as infection, allergy, or the presence of a foreign body.
  • Cosmetic Issues: When nasal granuloma causes deformity or aesthetic issues, excision is performed for cosmetic purposes as well as to restore the normal appearance of the nasal cavity and surrounding structures.

The decision to perform a nasal granuloma excision depends on the patient's symptoms, medical history, the underlying cause of the granuloma, and response to conservative treatments. It is usually recommended if conservative measures have failed to adequately alleviate symptoms, or if there are concerns of complications or malignancy.

How Does a Laser Excision of Nasal Granuloma Work?

Before

  • Patient assessment: A comprehensive patient evaluation is conducted that includes a review of the medical history and diagnostic tests such as nasal endoscopy and imaging studies such as CT scans.
  • Preparations: Pre-procedure instructions are given, which include fasting before the procedure as well as the discontinuation of certain medications that could increase the risk of complications.
  • Consent to the Procedure: The surgeon will discuss the associated risks, benefits, and alternatives, and obtain the patient’s consent.

During

  • Anesthesia: Depending on patient preference and the complexity of the procedure, the patient is given either a general or local anesthetic.
  • Endoscope guidance: The surgeon will use a nasal endoscope to visualize the nasal cavity and locate the granuloma accurately.
  • Laser excision: The nasal granuloma is removed precisely via laser beam so that it does not harm surrounding healthy tissue.
  • Hemostasis: During the procedure, the surgeon will secure hemostasis or control of bleeding with different techniques like electrocautery or the laser itself to help keep bleeding minimal and provide a clear field.
  • Closure: Depending on the size and location of the excised granuloma, the surgeon may have to close any incisions with dissolvable sutures or nasal packing to ensure that the area heals properly with minimal risk of postoperative bleeding or infection.

After

  • Recovery: The patient is observed in a recovery area until they are fully awake and stable.
  • Pain management: Pain medications may be prescribed to manage discomfort after surgery.
  • Follow-up care: Patients usually have follow-up appointments to make sure they heal well and address any concerns.
  • Activity: Patients are advised to avoid any strenuous activities and to follow any other instructions given by the surgeon.
  • Symptoms: Patients often experience relief from symptoms like nasal congestion or facial pain following the excision for granuloma.
