When is a Myringotomy Recommended?

A myringotomy procedure may be recommended in several situations:

  • Repeated ear infections: If an individual has frequent ear infections, particularly if they do not respond to other treatments, a myringotomy may be performed to relieve pressure and fluid buildup in the middle ear.
  • Persistent otitis media: For individuals who experience consistent inflammation and fluid buildup in the middle ear, myringotomy reduces the likelihood of further infections and potential hearing damage.
  • Eustachian tube dysfunction: If the Eustachian tube, a connection between the middle ear and the back of the nose, is unable to function normally, a myringotomy may be suggested to equalize pressure in the middle ear.
  • Serous otitis media: This condition, called otitis media with effusion, is marked by continued fluid buildup in the middle ear without any signs of infection. Myringotomy helps remove and drain the fluid, thus preventing complications.
  • Barotrauma: The buildup of severe pressure in the middle ear caused by rapid altitude or pressure change, such as during air travel or scuba diving, may require a myringotomy to relieve the pressure and prevent injury to the eardrum.
  • Glue ear: When thick, sticky fluid builds up in the middle ear it can cause hearing loss and repeated infections. myringotomy can drain fluid and improve hearing.

Myringotomy is typically considered when more conservative treatments fail to resolve chronic ear problems or when there is a chance of difficulties because of a buildup of fluid or changes in middle ear pressure.

How Does a Myringotomy Work?

Before the Surgery

  • Evaluation: An ENT (ear, nose, and throat) specialist will conduct a comprehensive evaluation of the patient’s condition to determine the need for myringotomy. The evaluation may include a physical examination, hearing tests, and possibly imaging studies such as a tympanogram or a CT scan.
  • Preparation: Before the procedure, the patient may be provided with instructions about how to prepare for surgery, including fasting for a specific period before surgery. If the patient is taking any medications, particularly blood thinners, may need to adjust or discontinue them before surgery to reduce the risk of bleeding.

During the Surgery

  • Anesthesia: The patient is usually given local anesthesia, but sometimes general anesthesia is also used, especially for young children or if other procedures are being done at the same time.
  • Incision: Using a surgical microscope or an endoscope for visibility, the surgeon makes a small incision in the eardrum (tympanic membrane) with a scalpel or laser. This cut allows fluid to drain from the middle ear and lets the pressure equalize.
  • Inserting Tubes (Optional): In some cases, a tympanostomy tube may be put in the eardrum through the incision. These tubes help keep air moving in and out of the middle ear and can be made of plastic or metal.

After the Surgery

  • Recovery: Following the procedure, the patient will be observed in the recovery area until the effects of the anesthesia have worn off. Most patients can go home on the same day after the procedure.
  • Postoperative care: Patients are typically discharged with instructions on postoperative care, which may include antibiotics or pain medicine. They may also be advised to not allow water to enter their ears and to follow up with their ENT specialist for the care or removal of their tubes (if used).
  • Healing: The small incision in your eardrum usually heals within a few days to weeks, depending on the individual and the type of tube used in the procedure. Ear tubes fall out on their own in 6-12 months in children and often 1-2 years in adults.

Follow-up: Patients typically have follow-up appointments with their ENT specialist to monitor healing, assess the effectiveness of the procedure, and determine if any further treatment is needed.

Risks and Benefits of Myringotomy

Risks
