Why do People Get Submandibular Gland Excision and Ranula surgery?

There are several reasons why people get submandibular gland excision and ranula surgery:

  • Chronic infections: Submandibular gland removal may be necessary in the case of persistent infections of the submandibular gland that do not respond to antibiotics or other conservative treatment measures.
  • Salivary stones (Sialolithiasis): They are stones that can form within the salivary glands or the ducts of the salivary glands. They can be removed by non-invasive methods if they are fairly small but if the stones are relatively large it might a surgery to alleviate pain and regain function.
  • Tumors: Benign or malignant tumors of the submandibular gland are treated with a gland excision to remove the tumor and prevent the spread of cancer.
  • Ranula management: A ranula is a mucous cyst in the floor of the mouth, often resulting from a blocked salivary gland duct. They can lead to discomfort and difficulty with speech and swallowing. Treatment may involve drainage or excision. In some cases, particularly when a ranula is large, recurrent, or associated with the submandibular gland, surgical excision of the ranula along with removal of the associated submandibular gland may be necessary to prevent recurrence.
  • Symptomatic relief: Each of these conditions could cause discomfort, pain, and swelling, and can make it difficult to eat or talk. The goal of surgery is to relieve these symptoms and enhance the patient's standard of living

When conservative treatments prove to be ineffective or the condition carries a risk of complications, surgery may be recommended. The choice of surgery depends on the severity of the condition, the presence of complications, and the patient’s overall health.

How Does the Procedure Work?

Before Surgery

  • Evaluation and diagnosis: The first step is to undergo a thorough examination with a doctor, potentially followed by certain tests such as ultrasound or MRI to correctly diagnose the condition and frame the best treatment plan.
  • Preparation for surgery: If the patient will receive general anesthesia, they may be instructed to not consume any food or drink for a certain period before the procedure. They may also be asked to make changes to their usual medications if necessary or quit habits such as smoking or alcohol consumption before the treatment.
  • Arrange for transportation and support: Arrange for a friend or a family member to drive you home after the procedure. Additionally consider having someone to support you during your recovery. 

During Surgery

  • Anesthesia: The patient is administered local or general anesthesia according to the requirement. 
  • Positioning: The patient is positioned on their back with the mouth open. The head will be tilted backwards to provide better access.
  • Incision: The surgeon makes an incision beneath the tongue to access the ranula. The size and location of the incision can very in each case. 
  • Removal: The surgeon will carefully disect and seperate the ranula from the surrounding tissues including any adjacent salivary glands or ducts. Once sepated the ranula is carefully removed.
  • Closure: Once the procedure is completed, the surgeon closes the incision with sutures or surgical glue.
